転移性乳がん患者由来CTCのPIK3CA遺伝子変異解析

 PI3K/Aktシグナル伝達の変異は、乳がんの発がんに不可欠な細胞プロセスの制御に関与しています。本プロジェクトの目的は、転移性乳がん(metBC)患者のCTC(Circulating tumor cell)におけるPIK3CA遺伝子変異を同定する方法の開発です。

 44名のCTC陽性metBC患者から得られた計57検体の血液検体を、CellSearchを用いて解析しました。濃縮したCTCからゲノムDNAを抽出し、増幅後、PIK3CA遺伝子のエクソン9とエクソン20の変異を解析しました。エクソン9の変異はE542KおよびE545Kの変異を、エクソン20の変異はH1047Rの変異を誘導し、PI3Kを活発化させます。変異はSNaPshot法を用いて検出しました。SNaPshot法とはPCR増幅と一塩基伸長法を組み合わせた方法です。

 SNaPshot解析は、MCF-7細胞株、T47D細胞株、SKBR-3細胞株それぞれから抽出したゲノムDNAを用いて反応系を検証しました。その後、血液検体の解析に応用しました。

 44名のうち7名の患者で、57検体のうち9検体の血液検体から、エクソン9/E545Kまたはエクソン20/H1047Rのどちらかの変異が計12個検出されました。検出された12個の変異のうち、エクソン9/E545Kの変異は6個(50%)で、エクソン20/H1047Rの変異は6個(50%)でした。CTC検出数は1~9748個でした。PIK3CA遺伝子のE542K、E545GとE545Aの変異は検出されませんでした。

 CTCのPIK3CA遺伝子変異解析は、例えばHER2標的療法に対するような、薬剤耐性について臨床的関連性を持っています。これも考慮すると、SNaPshot法を含む上記のアプローチは、血液検体から濃縮したCTCの変異同定を可能にし、治療に結びつくSNPs解析へと応用可能です。

治療前にCTCが検出された早期(非転移性)乳癌患者は予後不良

 これまでのCTC研究では、転移性がんにおけるCTC数が予後不良の予測因子となることを示してきましたが、非転移性がんや初期の乳がんにおけるCTC数と予後予測の関係を調べた研究はほとんどなされていませんでした。本論文は化学療法未治療のstageⅠ-Ⅲの非転移性乳がん患者におけるCTC数の測定と予後予測の関係の構築を目的としています。CellSearch®システムを用いてCTC数を測定した結果、302症例中73症例(24%)で1つ以上のCTCが検出されました。予後約35カ月間(中央値)をフォローアップしたところ、1つ以上のCTCが検出された場合、無増悪進行生存期間及び全生存期間は有意に不良となりました。本研究の結果、化学療法未治療の非転移性乳がん患者において、CTCはがんの再発・予後の予測と、治療方針決定に有効な情報となり得ることが示唆されました。
